A service report, also called a job sheet, is the record of a visit: why you were called, what you found, what you did and tested, and what's still to do, with the customer's acknowledgement at the end. What a service report should include

- The fault reported or the service requested.

- Materials and labour used
- Quantity and description, so they make it onto the invoice.
- Status and recommendations
- Completed, follow-up required, quote required or parts required, and what you recommend next.
- Customer acknowledgement
- The customer confirms the report was explained and the site was left safe, with their name, signature and date.
Four habits that make a job sheet worth keeping
- Write it up before you leave, while the customer is there to sign.
- Record test results as readings or pass and fail, not "all good".
- Photograph what you'll need to explain later: before, after, and anything you recommend fixing.

Common questions
Is a service report the same as a job sheet?
Here, yes: both names mean the record of what happened on a visit. If you use "job sheet" to mean the instruction before the job, you want the work order template instead.
Is this a compliance certificate?
No. It's a record of the visit for you and your customer. It doesn't replace any certificate, test report or notice your licence or your state requires for the work.
Can the customer sign it without printing it?
Yes. The acknowledgement at the end is made of PDF form fields: a tick box, the customer's name, a typed signature and the date. Your customer can fill them in with a PDF reader and send the file back.
Does it suit trades other than electrical?
The visit details, work carried out, test lines, materials and sign-off suit other trades too. The checklist items are written for electrical work, such as switchboard and RCD checks, so leave any that don't apply unticked.
